The following info is REQUIRED for each player:
- Player Name
- Player or Parent Email
- Player Phone Number
- Player Shirt Size
- Player Graduation Year
Payment: To complete this required online registration through our secure site, please have your Visa, MasterCard, Discover, or checking account information available.
Liability Waiver: Each player must have a parent/guardian complete the liability waiver form to turn in the first night of league play. Liability waiver forms can be printed from the league page or picked up at check in on the first league date. Waivers can not be submitted electronically as we need physical copies on site.
Refund Policy: Refunds will be issued if requested via email prior to the registration deadline and will be reduced by a $10 administrative fee. Refunds will also be reduced $8 per t-shirt or $18 per dry-fit shirt (if ordered). No refunds will be issued after the registration closes.
